Applications
All permit applications have been combined into one general
application form for the 2009-2010 permit year. Faculty/A&P,
Emeritus, Staff/CCS, Students, Visitors, Contractors, and
Vendors should use this application form.

UPGRADE/DOWNGRADE FORM
All upgrade/downgrade applications for the 2008-2009 parking
permit year were due by July 31st. No mid-year application
approvals will be made after this time.
DELIVERY PERMIT REQUEST FORM
Delivery permits are free of charge and must be requested
by a department chair, dean, or director. They must
be used in conjunction with an annual "A", "B", or "C" permit
and are valid in a designated loading zone for 30 minutes.
A Delivery Permit will also authorize any permit type to use
an "A", "B", or "C" space for 30 minutes if accompanied with
a Delivery Permit.

Student Payment Plan Form Students must sign a copy of the Student Payment Plan Terms of Agreement at time of permit purchase. Download a copy of the Terms of Agreement and bring to 160 Bevis Hall when purchasing your permit.

DISABILITY FORMS
Temporary Disability (1 month or less) and Handivan Rider Request Form
Form for one month for all faculty, staff, and students with a temporary injury or medical
condition restricting mobility and form for Handivan riders.

EVENT FORM
Event Parking Notification Form
Form for any on campus event that will bring in excess of
100 guests to campus. Forms may be e-mailed or faxed a minimum
of two weeks prior to the date of the event to the Transportation
& Parking Services Events Office at:

Driver Registration Form
The Driver Registration Form is required for all
employees that drive a university owned vehicle for any length
of time. In order to drive a university owned vehicle, this
form must be submitted beforehand. When completed, please

Vehicle Acquisition Form
This completed Vehicle Acquisition Form is required prior to acquiring a vehicle.
Gift-in-Kind Registration Form
The Gift-in-Kind Registration form must be completed and submitted to Transportation & Parking Services when the gift-in-kind vehicle is accepted.

INSTRUCTIONS:
- Complete SECTION A of the State Vehicle Disposal Form
- Contact the T&P Service Center (292-6195) to arrange for drop-off of your vehicle (or towing, if necessary)
- Please make sure the form (with SECTION A completed) accompanies the vehicle to the Service Center

On the second Wednesday of each month, Transportation & Parking Services will display vehicles that are eligible for intra-departmental transfer. Please call the T&P Service Center (292-6195) for more information and a current listing of vehicles that are available for internal reassignment.

FLEET SERVICES FORMS
Mileage Log - Usage
of University vehicles must be documented on a mileage/use log. Vehicles classified
as service vehicles (marked cruisers, service trucks and vans, etc.) are exempt
from this requirement. This form may be used, or each department may elect
to develop their own. Forms should be retained by the department and provided
upon request of the auditors.
Vehicle Pre-Trip
Inspection Form - A sample document that allows you to record the condition
of a vehicle prior to daily use or whenever drivers are changed. (Click here for
the Van Pre-Trip Inspection Form.) This form should be retained by the department
for internal use only.
Voyager
Credit Card Application -
University vehicles should be fueled at University facilities when feasible.
The Columbus Campus fueling site is located at the Service Annex, 2578 Kenny
Road. If use of the University's fueling station is not feasible, or when
the
vehicle is to be used off campus or on extended trips, departments may apply
for a VOYAGER
Credit Card. Use of the credit card is restricted to University vehicle
fuel expenses and repairs. No other University credit card should be
used to purchase fuel to avoid federal fuel tax charges.
